Good Clean Dog Service Agreement

I, the undersigned, agree to the following terms and conditions regarding grooming services provided for my pet by Good Clean Dog (“GCD”):

GENERAL AGREEMENT

Good Faith Estimate: GCD has provided a price estimate to me for services based on information that I provided during the scheduling process. I understand that GCD may refuse service or may need to adjust the service price should my pet arrive with different characteristics than those that I previously provided.

Health & Condition of Pet: I confirm that my pet is healthy, and I agree to always disclose any known medical conditions, allergies, or special needs. If my pet has fleas or other parasites, I understand additional charges may apply for treatment or I may be asked to keep my pet home until healthy. I confirm that my dog is currently up to date with Rabies, Distemper and Bordetella vaccines and I will continue to maintain these vaccines on an annual basis. I understand that these vaccinations are required and that grooming sessions may be terminated without proof of vaccination.

Pre-existing Conditions: I will keep GCD informed of any pre-existing medical conditions, injuries, or sensitivities my pet has that may affect the grooming process. I understand GCD will not be held responsible for exacerbating any existing conditions.

Aggressive or Anxious Pets: If my pet shows signs of aggression or extreme anxiety, I understand that the grooming session may be terminated for the safety of my pet and the staff. I will be informed if additional precautions or measures are necessary to proceed with grooming.

Matting & Shaving: I understand that if my pet’s coat is severely matted, there may be risks of skin irritation, cuts, or nicks during the de-matting process. In severe cases, shaving may be required to ensure my pet’s comfort and health. I release GCD from liability for any consequences arising from the de-matting or shaving process.

Veterinary Treatment: In the event that my pet requires veterinary care while at GCD, I agree to cover all costs for treatment. I authorize GCD to seek and provide veterinary care from my designated veterinarian or a veterinarian of GCD’s choice. GCD will make reasonable efforts to contact me before seeking veterinary care.

Photographs & Media: I consent to the use of photographs or videos of my pet during grooming for promotional purposes by GCD.

Fees & Payments: GCD requires a valid credit card authorization in order to schedule an appointment. I understand that, should I opt to pay via check, GCD reserves the right to process payment via the credit card provided in the event that the check is returned.

Right of Refusal: GCD reserves the right to refuse service to any pet or client for any reason.

NO-SHOW / CANCELLATION FEE

To ensure the best service for everyone, we require that our customers agree to the following no-show and cancellation policy:

GCD kindly ask that customers notify us at least 24 hours in advance if they need to cancel or reschedule an appointment. This gives us the opportunity to accommodate other pets on our waiting list.

I understand that if I miss or attempt to reschedule my grooming appointment without providing at least 24 hours prior notice, I will be charged a fee equal to the amount of the service that was scheduled.

Payment will be charged automatically via the credit card authorization provided when the appointment was scheduled.

In the event of inclement weather, a family emergency or any other uncontrollable circumstance, GCD has the discretion to waive the fee within the 24 hour period.

RELEASE OF LIABILITY

I acknowledge that pet grooming can be stressful for some pets and may involve risks such as nicks, cuts, irritations, or other minor injuries. I understand that, even though I may specify that I prefer my pet to be crated, GCD is largely a cage-free, open play environment which may result in the transfer of communicable illness and/or injuries such as broken nails, abrasions, cuts, etc. I release GCD from any liability in the event of accidental injury or stress-related issues.
